PDF QUESTION FORM TO PRINT FOR PILGRIMS

Sponsors obtain information from each pilgrim using the PDF questions you can obtain from the ‘PDF of Questions’ button on this page. As a sponsor, this is how you get the registration information you need to fill out an application.

  • Click the “PDF of Questions” button to download the template. After first talking with them about the walk, then obtained a ‘yes’ from them. The next thing to do is to print out this PDF form and have them fill it out completely. Use email if you want to paste the questions and get their info back in electronic form. .

  • They must return their answer to you as sponsor. There are confirmation questions that must be answered.

SPONSORSHIP GUIDELINES

You came to this page after reading the sponsorship page and watching the sponsorship video. Sponsors should know the guidelines and rules for sponsorship in detail and have watched the video.

The details of submitting a sponsorship application is listed below. It involves reviewing the PDF page of questions to get returned from the pilgrim and a link to both the application form and the way to submit the Sponsorship Registration Fee.

REGISTRATION - SPONSOR SUBMITS FORM

After receiving back the information of the PDF questions above, sponsors should submit an on-line application. It is the responsibility of the sponsor to use that

After the sponsor receives the information back from the PDF above, the sponsor (and only the sponsor) should enter all that info into our electronic process.

Please refer to the sponsorship page often, linked by the top menu, during the four months prior to the weekend retreat. Ensure you have paid the registration fee and done all other steps.

Your application is due before the second team meeting, approximately six weeks before the Walk.

REGISTRATION FEE

Please use this link to pay the sponsorship registration fee of $35 after your pilgrim has been accepted into this Walk. This fee is $45 if paid after the second team meeting.
